If you do not agree with any part of these Terms, you must immediately discontinue use of this website and our services. These Terms apply to all visitors, clients, employers, attorneys, and any other parties who access or use our website or services.

These Terms of Use should be read in conjunction with our Privacy Policy, HIPAA Notice of Privacy Practices, and SMS Terms & Conditions, all of which are incorporated herein by reference.

LHID Labs (Lasting Health Impact LLC) is a CLIA-certified drug testing collection site located at 1558 Marietta Highway, Suite 200, Canton, Georgia 30114. We provide drug and alcohol specimen collection services for workplace, DOT-regulated, court-ordered, legal, and personal testing purposes.

Specimen analysis is performed by contracted SAMHSA-certified laboratory partners. Non-negative results are reviewed by licensed Medical Review Officers (MROs). LHID Labs is a collection site — we do not perform laboratory analysis of specimens on-site.

Our services include, but are not limited to:

• Urine, oral fluid, and hair follicle specimen collection
• DOT-regulated testing under 49 CFR Part 40
• Court-ordered, probation, parole, and legal testing
• Pre-employment and random workplace testing
• Third Party Administrator (TPA) and consortium management services
• Mobile and remote collection services
• Secure online results delivery via our HIPAA-compliant portal

You must be at least 18 years of age to use our website and schedule testing services independently. Minors may be tested only when accompanied by a parent or legal guardian, or pursuant to a valid court order or employer authorization.

If you create an employer or portal account with LHID Labs, you are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account. You agree to notify us immediately at (844) 984-5443 if you suspect any unauthorized use of your account.

Employer accounts may only be used by authorized representatives of the registered business entity. You represent that you have the authority to bind your organization to these Terms when registering for or using employer services.

Drug test results and related health information collected in connection with our services constitute Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). All PHI is handled in accordance with our HIPAA Notice of Privacy Practices.

Results are disclosed only to authorized parties as directed by the donor, required by court order, or mandated by applicable federal or state law, including DOT regulations. Drug testing results related to substance abuse treatment programs may be subject to additional protections under 42 CFR Part 2.

You agree not to use this website or our services to:

• Submit false, misleading, or fraudulent information, including false identification at the time of collection
• Attempt to adulterate, substitute, or otherwise tamper with a specimen
• Interfere with, disrupt, or compromise the security or integrity of our systems or services
• Harvest, scrape, or collect data from our website without express written permission
• Use our services for any unlawful purpose or in violation of any applicable federal, state, or local law
• Impersonate any person or entity, or misrepresent your affiliation with any person or entity
• Transmit any unsolicited commercial communications (spam) through our contact forms or systems
• Attempt to gain unauthorized access to any portion of our website, portal, or systems

Specimen tampering or fraud may constitute a federal or state criminal offense and will be reported to appropriate authorities where required by law.

All content on this website — including text, graphics, logos, images, page design, and service descriptions — is the property of Lasting Health Impact LLC (LHID Labs) or its content suppliers and is protected by applicable copyright, trademark, and other intellectual property laws.

Our total liability to you for any claims arising under these Terms shall not exceed the amount you paid to LHID Labs for the specific service giving rise to the claim in the six (6) months preceding the claim.

Some jurisdictions do not allow the exclusion or limitation of certain damages, so the above limitations may not apply to you.

You agree to indemnify, defend, and hold harmless Lasting Health Impact LLC (LHID Labs), its members, officers, employees, and agents from and against any claims, liabilities, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or related to:

• Your violation of any applicable law or regulation
• Any fraudulent, false, or misleading information you provide

Any dispute arising out of or related to these Terms or your use of our services shall be subject to the exclusive jurisdiction of the state and federal courts located in Cherokee County, Georgia.

You agree to submit to the personal jurisdiction of such courts and waive any objection to the exercise of jurisdiction over you by such courts.

LHID Labs reserves the right to update or modify these Terms of Use at any time. When we make changes, we will update the "Last Updated" date at the top of this page. We encourage you to review these Terms periodically.
