We may use and disclose your PHI to obtain payment for services provided. For example, we may submit billing information to your employer or a third-party administrator.
We may use and disclose your PHI for our internal operations, including quality assurance, staff training, legal services, and business management activities.
We will disclose your PHI when required to do so by federal, state, or local law, including in response to valid court orders, subpoenas, or DOT regulatory requirements.
We disclose test results to employers, courts, attorneys, probation officers, drug court coordinators, and other authorized parties as directed by you at the time of testing or as required by the terms of your testing authorization or court order.
For uses and disclosures not described in this notice, we will obtain your written authorization. You may revoke any authorization at any time in writing, except where we have already acted in reliance upon it.
Drug testing results that relate to substance abuse treatment programs may be subject to additional federal confidentiality protections under 42 CFR Part 2, which provide stricter privacy protections than standard HIPAA rules. We comply with all applicable regulations.
