What Is a DOT Physical?

The Department of Transportation (DOT) requires all commercial motor vehicle (CMV) operators to pass a physical examination performed by a certified Federal Motor Carrier Safety Administration (FMCSA) Medical Examiner. This is a federally mandated health screening — not a general checkup.

The exam ensures that commercial drivers are physically and mentally fit to safely operate large vehicles on public roads. Without a valid Medical Examiner's Certificate (MEC), a CDL driver cannot legally operate a commercial vehicle.

A DOT physical typically takes 30–60 minutes. Here's what happens from the moment you call to when you walk out with your certificate.

Health History Intake

You'll complete the FMCSA Medical Examination Report Form (MCSA-5875), disclosing medical history, medications, and any conditions relevant to safe driving.

Physical Examination

The certified Medical Examiner evaluates vision, hearing, blood pressure, urinalysis, cardiovascular health, neurological function, and overall physical fitness.

Certificate Issued

If you pass, the Medical Examiner's Certificate (CDL MEC card) is issued same-day. Results are submitted electronically to the FMCSA National Registry within 24 hours.

What to Bring to Your Exam

Having your documents ready ensures a smooth exam. Please bring all of the following to your appointment.

Government-Issued Photo ID

Driver's license, state ID, or passport. Must be current and valid.

Current Medications List

All prescription and over-the-counter medications including dosages. Bring bottles if possible.

Eyeglasses or Contact Lenses

If you normally use corrective lenses for driving, wear or bring them to your exam.

Relevant Medical Records

Records for diabetes, sleep apnea, heart conditions, epilepsy, or any condition that may require a waiver or exemption letter from your treating physician.

Hearing Aids (if applicable)

Bring and wear your hearing device. The Medical Examiner will note their use on your certificate.

How Long Does
a DOT Physical Last?

A DOT Medical Examiner's Certificate is typically valid for up to 24 months. However, if you have a condition such as controlled hypertension, diabetes, or sleep apnea, your Medical Examiner may certify you for a shorter period (3, 6, or 12 months) to allow for monitoring.

Do not wait for your certificate to expire. Driving with an expired MEC is a federal violation. We recommend scheduling your renewal exam 30–60 days before your expiration date.

Can I fail a DOT physical?
Yes. Certain conditions may result in temporary or permanent disqualification, including uncontrolled blood pressure, certain vision impairments, or active substance use disorders. Your Medical Examiner will discuss options including physician letters, waivers, or exemptions.
Is this the same as a drug test?
No. A DOT physical does not include a drug or alcohol screen. The urinalysis checks for glucose and protein only. A separate DOT drug test is required by FMCSA — we offer these as a convenient bundle.
How long does the appointment take?
Most DOT physicals are completed in 30–60 minutes. If you are bundling with a drug test, plan for approximately 75–90 minutes.
What if I take blood pressure medication?
Controlled hypertension is generally manageable. Take your medication as prescribed on the day of your exam, and bring your medication bottles. Certification period may be shortened to allow monitoring.
